Once you have completed your form, please email it to coaches@svaa.org. Forms will be processed in the order they are received.

If you have any specific requests, please include them in the “Additional Information for Office” section. For example, if you coach two teams and need to coordinate practice times, please note that on your form.

Please return only one form per team.

Practices begin the week of November 30. There are no practices when schools are closed.
